Your plan is led by one of our nurse practitioners, Christina Martinez, FNP-C, or Luzelle Cabuntala, FNP-BC, with Sarah Hoffman, RN, BSN, on the care team. Dr. Ryan Peterson, our medical director, oversees care at the center.
